            This is a self-report measure administered only to ages 18-85 as a CAT. In addition, a fixed-length self-report form is available for ages 18-85 as a supplemental measure. Items administered use a 5-point scale, with options ranging from âstrongly disagreeâ to âstrongly agree,â or from ânot at allâ to âvery much.â The survey is scored using IRT methods.
